Complete the registration form and return it along with your full payment to U.S. Cheer, PO Box 290037, Columbia, SC 29229 by the deadline. Team registration should be in one check only. Any payments that are not received within four weeks of our event must be in a form of certified funds such as money order or cashier’s check. Please fully complete each section so that we do not have to return the form. Be sure to make a copy of this form for your records.
